Children’s Home Healthcare Ventilator and Tracheotomy care is designed to provide the support and care that enables families who are facing the challenges of caring for a patient on a ventilator or with a tracheotomy to confidently and safely care for their loved one at home. We provide hourly or around the clock services in a well-coordinated, family centered approach to meet the complex needs of these patients and their caregivers.
Our nurses are specially trained and experienced to care for ventilator dependent and tracheotomy patients in their homes. Before services commence, one of Children’s Home Healthcare’s Registered Nurses, in consultation with the patient’s attending physician and family members, will visit the patient’s home, to develop a comprehensive plan of care. In conjunction with family members, who may be active participants in the patient’s care, our primary goal is to help the patient attain his or her highest level of well-being.
Once services commence, periodic home supervisory visits are conducted by Registered Nurses to assure the services are being provided according to the plan of care. Furthermore, written daily patient progress notes and outcome reports are made available to the patient’s physician, authorized family members, and case managers. In the event that an issue or question arises, the company’s office staff and nursing supervisors are available 24 hours a day, 7 days a week, 365 days a year.
