The Medically Fragile Waiver Program (MFW) is for individuals who live at home with a medially fragile condition and developmental disability. The program provides for a third part RN Case manager and the private duty nursing services of an RN or LPN.

To apply for the MFW program, contact the MFW Manager at 505.841.2913 or CLICK HERE to learn more about the program. There is a waiting list for the MFW program so also ask about receiving EPSDT services.

With both the EPSDT and MFW Programs a third party RN Case Manager will be assigned. They and the family will work with Children’s Home Healthcare to set up needed private duty nursing care.
