You will have a dedicated case manager who will keep you informed, answer your questions, and coordinate with the clinical team to ensure your child’s care plan is being followed. You will also have a dedicated scheduler to help you manage the calendar and schedule of your child’s nursing care. We also offer a 24/7 on call line for families and are available by phone whenever needed.
